| 一、 | 本案申請資訊說明如下： |
| (一) | 申請資格：輔導對象為義務教育階段學生，因家庭遭遇變故而中途輟學，經追蹤輔導返校而無法適應就學環境或因家庭功能不彰而有中輟之虞，經家長或監護人同意接受輔導就讀者，並經基隆市復學輔導就讀小組審查符合資格。 |
| (二) | 申請時間： |
| １、 | 年度招生：114年5月1日至5月20日，申請對象為設籍於台澎金馬之5-8年級學生。 |
| ２、 | 第一次期中招生：114年10月1日至10月15日，申請對象設籍於台澎金馬之6-9年級學生。 |
| ３、 | 第二次期中招生：114年12月20日至115年1月35日，申請對象為設籍於台澎金馬之6-8年級學生。 |
| ４、 | 請將相關資料備齊並逕寄至基隆市中山高中國中部大德分校慈輝班(以郵戳為憑，逾期不候)。 |
