一、 檢送「教育部生命教育特色學校及績優人員遴選表揚實施計畫」，如擬推薦「特色學校」、「績優人員」、 「特殊貢獻人員」、「終身奉獻人員」，請依「初選薦送名額一覽表及填寫說明」填妥相關薦送表件，於115年6月1日(星期一)前送交至本局，逾期不予受理；相關資料不予退還。
二、 相關表件格式電子檔案請逕至教育部學生事務及特殊教育司網站「重要業務專區/學生事務/業務資料下載/素養教育專區/生命教育」下載使用(網址：https://reurl.cc/MRRVZW)，標題「115年教育部生命教育特色學校及績優人員評選薦送表件」。
三、 請審慎查核受推薦人員不得曾有違反教師法，包含尚在調查、或經議決解聘、不續聘、停聘或資遣之情形。
